FROM LAST MESSAGE SENT FROM Wiiliam Monroe ….As of 9/29/2017..I file a charge with the EEOC OF DISABLITY DISCRIMATION AND RETALATION AGASINT MY ENPLOYER…TGE MEDIATION WAS PASS OVER INTO INVESTIGSTION AND THE RESULTS OF THAT WAS A RIGHT TO SUE. GRANTED TO ME….I HAVE 90 TO PURSUE MY CASE IN FEDERAL COURT SEEING THAT ECIDENCE OF DISCRIMATION WAS FOUND…THANK YOU.
I went on a six motnths short term paid disability leave from my employer due to my start of dailysis treatment. ..On 06/21\2017 i was mailed a letter from my employer stating that in order to return to work a certification from a physician is neededof my acommadtions…On 08/08/2017..i brought a physician letter of return to work explianing my condition diagnosis.( Dailysis ) Ihave a disabilty and my restrictions less work hours in seeing that im a part time employee it was a acommadations of less work hour 14 per week 2 days per week..Tues and Thurs…not to be an interference with my treatment..on 08/09/2017…i was told by my manager.that i could not return to work unless i was 100% restrictions free…i called the employee hotline and made a complaint about this matter discrimation and retalation seeing that i. have complain berfore of senority being overlook and current disability..on 08/112017..I received a call from my employers acommadation coord expressing why i cannot work Saturdays…my explanation was its an designated day for emergencies for treatment…as all treatment centers have…the acomm cood requedt to me that she would like to ask my physician some questions i applied and when the letter came of a request attach was a voluntary authorization release form stating to realease all infomation of my information frim my healthcare provider..my physicians care….clinics visit..my scientistic labs…my medical summary diagnosis and prognosis and or casual functions …i took the letter to my physician a was advised that all my medical info would ne given to them..so i did not sign the form and my physician sent the exact return certification dated 08/14/2017…i left a phone message to the employer acomm coord that i incline not to sign the form due to my right to pirnacy between patient and physician. I received a call from the acomm coord that they cannot bring me back to work unless i sign the form…i then wrote a letter the the coord stating that i would mot voluntary sign the form…from the month of 08 to 09 calls went back and forth on 08/26/2017 i received a letter from my employer stating that the complaint of discrimation and retaliation had been resolved and thank for present this issue to them…My employer then wrote to my physician in attemp to retreive my entire medical information without my consent knowingly that this is an ADA VIOLATION..my physician wrote a letter back declining the information of my mefical records. .As phone message and conversations thur 08 and 09 of 2017 continued between me and my employer…it came to a harssment of letters sent to two letter forcing me to sign the form if not i will be put on unpaid leave if absence. .
Im coming to usa for visit visa so iwant to ask if ican convert it to work.permit
